5Essential Parent Survey

Dear Parent/Guardian,

With the driving force to improve our school, our school programs and my leadership, we are participating in the 5Essentials Survey, a highly effective tool developed by UChicago Impact at the University of Chicago.  Hopefully, the results of this school-wide survey will provide me with critical and comprehensive information that can be used to deepen the impact of our school improvement plan.

Archdiocesan Initiative School Plan Survey

Dear Parent/Guardian,

The Archdiocese of Chicago and all of its schools are engaged in an ongoing continuous improvement process, of which all stakeholders are an integral part.  The Archdiocese is using the AdvancED Parent Survey as a means to elicit your opinions to help target areas for the school system to improve and areas to continue to be effective.  The five sections on the survey represent the five AdvancED Stardards for Quality School Systems and provide a means for learning about parent expectations of a high quality school system.  Your opinion is important to us, and we ask that you take the time to complete this survey.  The online survey will be available from Friday, January 24 until 8:00pm Tuesday, January 28.  In order to complete the survey, please click on this link:

 If you need the paper copy of the survey, it can be obtained at the school office however, the deadline for its completion still remains Tuesday, January 28, 8:00pm.
